- Common in children after laminectomy
- Aetiology
- Decompression leads to skeletal defficiency especially facet joint, ligamentous instability & muscular imbalance
- Gravity tends to produce a kyphosis in the C spine & Lordosis in the lumbar spine
- Instability is partly linked to horizontal nature of the facets
- Risk is 100% at c-spine
- Risk is 30% for t-spine
- Risk is 0 for the l-spine
- Prevention by considering preservation of the facet joints or fusion at the time of surgery
- Treatment
- Severe deformity requires an anterior posterior procedure
