Post Laminectomy Kyphosis

  • Epidemiology
    • Common in children after laminectomy
  • Aetiology
    • Decompression leads to skeletal defficiency especially facet joint, ligamentous instability & muscular imbalance
    • Gravity tends to produce a kyphosis in the C spine & Lordosis in the lumbar spine
    • Instability is partly linked to horizontal nature of the facets
    • Risk is 100% at c-spine
    • Risk is 30% for t-spine
    • Risk is 0 for the l-spine
    • Prevention by considering preservation of the facet joints or fusion at the time of surgery
  • Treatment
    • Severe deformity requires an anterior posterior procedure
